Dr Shackcloth: And another question. Did you use anything fairly subjective looking at gastric dilatation? Did you try any radiological methods or looking at factors such as paracetamol absorption to give you a much better quantitative measure of gastric emptying? Dr Renaud: To measure the gastric distension? Dr Shackcloth: That is fairly subjective. You could use radioactive tracers or paracetamol, which is absorbed purely in the small bowel. That would give you a much more quantitative measure of gastric emptying. Dr Renaud: We didn’t. Dr M. Mueller (Vienna, Austria): I really like the idea of an irritation of the vagus nerve that can cause gastric distension. For me, it is hard to understand or to believe that irritation of the vagus nerve on one side leads to the consequences you have shown. There is an old operation which was done in the era before we had all the drugs to block acid production, which was selective proximal vagotomy, and we learned during these operations that there is a huge network of the vagus nerve on both sides of the oesophagus and also of the stomach. So irritation of the vagus nerve on one side should not really lead to these severe consequences. So for me it is difficult to believe that this is really the main reason for the problem. Dr Renaud: Maybe it is an acute state, maybe the symptomatic time to compensate the problem with the other vagus nerve. I don’t know. But in our review, we have seen that and it is the only explanation we have for the movement. Dr J. Kuzdzal (Krakow, Poland): I did not fully understand your technique for the measurement of gastric distension. If the stomach is distended by the fluid content, we don’t see it on the chest X-ray and we in fact cannot assess the distension with the fluid. However, regarding the air distending the stomach, it is also quite often seen that there is a superimposition of the gas filling the colon on the gas bubble in the stomach, and it is also difficult for me to discern these two situations. I doubt whether this kind of measurement is really accurate. Dr Renaud: Concerning the dilatation of the bowels, if we had a doubt between the stomach or the bowels, these patients were excluded. For us, we ensure that all the measures that have been done are measured down on the stomach. And concerning the measure, of course, if it is distension with liquid, we were not able to measure it.

Introduction to The Changing Nature of the Business Cycle
Fluctuations in economic activity over the course of the business cycle imply signifi cant welfare costs. Most obvious are the costs associated with periods of relatively weak activity, when productive resources are under-utilised, and consumption is below par.1 Periods of relatively strong activity can also be costly since they imply an ineffi cient bunching of activity, consumption and invest...
متن کاملMeasuring Underlying Inflation
The primary goal of monetary policy in most countries is low infl ation. And a major consideration for the monetary policy decision is the forecast for infl ation over the next couple of years. One important input into these forecasts is a judgment on the current level of infl ationary pressures. But, unfortunately, the data for headline infl ation released each quarter (in Australia) or each m...
متن کاملOptimal Monetary Policy in a Financially Fragile Economy
This paper studies optimal monetary policy in an economy where rms use external funds to nance operational costs. It is shown that direct and indirect cost channels for monetary policy arise when rms can default on borrowed funds. The direct cost channel calls for milder contractions in the face of inationary pressures. The indirect cost channel, on the other hand, encourages policy conserv...
متن کاملComputational Study of Anatomical Risk Factors in Idealized Models of Type B Aortic Dissection.
OBJECTIVE/BACKGROUND Several risk factors have been identified in type B aortic dissection (TBAD), namely tear size, location, patency and number, and false lumen (FL) location. However, the individual impact of each of these factors is poorly understood. The impact of these factors was investigated using computational fluid dynamics (CFD). METHODS Fourteen idealized models of chronic TBAD we...
متن کاملIn ationary reheating classes via spectral methods
In ationary reheating is almost completely controlled by the Floquet indices, k. Using spectral theory we demonstrate that the stability bands (where k = 0) of the Mathieu and Lam e equations are destroyed even in Minkowski spacetime, leaving a fractal Cantor set or a measure zero set of stable modes in the cases where the in aton evolves in an almost-periodic or stochastic manner respectively....
متن کاملCosmologies with Two-Dimensional Inhomogeneity
We present a new generating algorithm to construct exact non static solutions of the Einstein eld equations with two-dimensional inhomogeneity. In nite dimensional families of G1 inhomogeneous solutions with a self interacting scalar eld, or alternatively with perfect uid, can be constructed using this algorithm. Some families of solutions and the applications of the algorithm are discussed. Re...
